Project: Discharging chronically ill patients, what are the effect of collaboration between hospital and primary health care?

Coordination of services between hospitals and the primary health care is emphasized in the Coordination Reform (”Samhandlingsreformen”). Our aim is to critically review literature that evaluates the effect of a defined coordinated action between hospitals and the primary health care, and the specific action must be targeted to the discharge of chronically ill patients. Effects of the coordinated action that will be reported are related to patient outcomes, e.g. the impact on health and level of satisfaction, to the health services, e.g. resource use, length of stay or readmissions, or to skill development and working conditions both at hospitals and in the primary health care.
